All telephone and computer related service requests are now being
handled by Telecommunications and will be processed through the RT
system.
Use this service request form for all requests including but not limited
to: moving/installing phone and computer lines, installing new jacks
(phone and/or data), ordering headsets, adding voicemail, making changes
to menus, ordering handset/base cords and upgrading phones.

This form does require a system ID. Non-USM entities that do not have a
PeopleSoft/Maine/Street username and password will need to contact
Telecommunications directly in order to submit a service request form.
