Human Performance Lab
 Students enrolled in the Exercise Physiology Major must provide formal documentation indicating that the following requirements have been met before they are allowed to attend the designated courses.
Proof of compliance deadlines for each semester:

**Failure to submit current documentation by the due date will result in the students immediate removal from the course.**
| SPM 431 |
| American Red Cross CPR for the Professional Rescuer |
provide proof of current American Red Cross CPR for the Professional Rescuer or it's equivalent. |
| SPM 460 |
| Hepatitis B Vaccinations (1 & 2) |
provide documentation of receiving the first two of three required Hepatitis B immunization injections. If you choose not to be Hepatitis B immunized, you must read, complete and sign the form entitled Hepatitis B Declination Assumption of Risk and Release prior to attending class. |
| Health Insurance |
provide proof of insurance coverage equal to or better than the basic USM Student Health Insurance Plan. Documentation should include a copy of a current health plan card with the name of the insurance carrier, policy number, etc. |
| American Red Cross CPR for the Professional Rescuer |
provide proof of current American Red Cross CPR for the Professional Rescuer or it's equivalent. |
| SPM 475 |
| Health Insurance |
provide proof of insurance coverage equal to or better than the basic USM Student Health Insurance Plan. Documentation should include a copy of a current health plan card with the name of the insurance carrier, policy number, etc. |

| Hepatitis B Vaccination (#3) |
provide documentation of receiving the third injection in the Hepatitis B immunization series. |

Immunizations may be obtained at your physicians office or University Health Services.
