Please note that the MS Word form can be filled out in Word,
saved, then sent via an e-mail attachment to IRB. The PDF form can be filled out and printed, then sent via inter-campus mail to
IRB. You will not be able to save changes to the PDF version of the form locally unless you are running a licensed copy
of Adobe Acrobat Professional. Adobe Acrobat Reader will not allow you to save changes to the document.
