Documentation Guidelines
In order to fully evaluate requests for accommodations
or auxiliary aids and to determine eligibility for services, the
Office of Support for Students with Disabilities (OSSD) needs documentation
of the student’s disability.
The documentation must include evidence of both a disorder and
related current, significant, functional limitations. Designed to
provide equal access, accommodations are provided in response to
the functional limitations of a disability as they clearly intersect
with academic demands. Accommodations do not include interventions
that are remedial, needed for personal study and care, or designed
to ensure desired outcomes.
Required documentation should be provided by an appropriate and
qualified medical or other licensed professional evaluator under
separate cover on practice letterhead. Copies of medical records
will not be accepted.
