2013 USM Summer Youth Music and Theatre Camps Registration Form

Registrations are accepted on a first come, first served basis. Some sections fill quickly; early registration is advised.

Note: After registering, a receipt will come to you by e-mail, but not immediately. Please submit the form only once, or duplicates will be created.

Instructions for registering:

1-a. Scroll down to complete the web form with basic registration information and submit. Once you submit the form, you will see a receipt for your submission with further instructions about payment that should be printed. No payment will be required at this time. Online registrations are time-stamped. Soon after you will receive a confirmation e-mail.

1-b. Phone registrations will also be accepted at the Music Box Office, (207) 780-5555. If there is no live phone attendant, please do leave the basic registration information as a message. Phone messages are time-stamped, but no immediate receipt can be given. Confirmation calls will be made as soon as possible.

2. Registrations that are mailed will also be accepted, but the date and time of receipt cannot be guaranteed. Confirmation calls will be made as soon as possible.

3. A deposit of $50 must be made within seven days to validate each summer camp registration. Deposits/payments may be checks or money order by mail, credit card via telephone (Visa/MC/Discover), or in person by appointment. Please, never send payment information by e-mail.

This is an optional field, and can be answered later. Because of room sizes, not all requests can be granted. We do our best to accommodate requests.

Recap the dates and ages

Choose "theatre" only for Theatre Academy. Music camps registrations must show only one primary instrument. Anyone who plays a secondary instrument may include that in the comments, if you choose. All band camp registrants may also choose piano, guitar, bass guitar or double bass, but only if their primary instrument is a wind or percussion instrument. Piano, guitar, bass guitar or double bass can only be a primary instrument for jazz camp.

This e-mail address should be a parent address that is regularly used. Important camp information and updates will be sent by e-mail to parents, and it's very important that it is received. Student and secondary e-mail addresses can be added to the comments, if you like.

We do not ask for payment with this form. In order to validate this registration, within 7 days, we must receive a deposit of $50 either by cash, check or credit card. We will work with you to complete your payment by your preferred method. We do not charge you a fee for credit card payments, however cash and check payments cost us less to process. Final payments must be received at least two weeks prior to the start of camp.

Answering this question will help us with future communications about our summer camps.

Due to space and ensemble limitations, submission of this registration form is not a guarantee of camp enrollment. You will receive enrollment confirmation from camp staff. Please mention here if there is anything unclear, or additional information you would like us to have at this time. Once your registration is confirmed, we will ask you for the more detailed information we will need for your camper to have a safe and enjoyable experience. Include in this field any secondary instruments, and any other questions you may have.
