Any student trying out for a sport at USM
for the first time will need to have a physical
examination at UHCS. Please download the first
time athlete forms located below. Read
through each form and carefully fill out each form.
Please remember to bring all these forms with you to the
health center at the time of the clinic along with your
insurance card and a form of payment.
