Dental coverage now available through Cross
Insurance
The University of
Southern Maine Student Health Insurance Plan has been
developed especially for University of Southern Maine
students and their eligible dependents. (Eligible dependents
include a lawful spouse, same or opposite domestic partners,
and/or dependent children under age 19). The Plan provides
coverage for illness and injuries that occur on and off
campus and includes special cost-saving features to keep the
coverage as affordable as possible. Specifics about

The 2008-2009
Plan

Coverage becomes effective
12:01 am on September 1, 2008 and will terminate 12:01 am
of September 1, 2009

Coverage purchased in the
fall semester will remain in effect for the entire year.

New spring semester students
may elect to purchase coverage which will become effective
at 12:01 am January 1, 2009 and terminate at 12:01 am
on September 1, 2009

IMPORTANT!
If you choose not to use services provided by UHS staff, services
through other health providers are available. You can
locate a provider through
Cross Insurance.
However, there will be a $10
office copay per visit. The student is responsible for
submitting charges to Aetna Student Health for
reimbursement. Referrals are not required for treatment of
Emergency Medical Conditions as defined on page 25 of the
insurance plan booklet or for routine GYN care.
What about referrals?
While a referral is not necessary
to receive benefits, students' health
care needs can be best satisfied when an organized system
of health care providers at University Health Services manages
the treatment. If you are enrolled in the Student Health
Insurance Plans, it is to your advantage to first seek treatment
at University Health Services and/or
University Counseling Services
to reduce your out-of-pocket
expense. If you choose not to take advantage of the services
provided by UHS/UCS, you will be responsible for paying charges
not covered under the terms of the policy.
