Patient Bill of Rights
- Patients and providers are treated
with mutual respect, consideration, & dignity.
- Patients receive health care without
out fear of discrimination of any kind.
- Patients are afforded appropriate
privacy.
- Patients statements and records are
treated confidentially, and patients must approve their
release in writing, except as required by law.
- Patients are provided to the degree
known with complete information concerning their diagnosis,
evaluation, treatment, and prognosis.
- Patients are given the opportunity
to participate in decisions involving their health care,
except when such participation would cause life-threatening
illness or injury to the patient or other individuals.
Patients have the right to information
concerning:
- Patient rights, responsibilities,
and conduct.
- Available services at University
Health Services.
- Instructions for after-hours and
emergency care.
- Fees for services at UHS.
- Payment policy and options.
- Patient's right to refuse participation
in prescribed treatment plan.
- UHS provider credentials.
