Patient Bill of Rights

  • Patients and providers are treated with mutual respect, consideration, & dignity.
  • Patients receive health care without out fear of discrimination of any kind.
  • Patients are afforded appropriate privacy.
  • Patients statements and records are treated confidentially, and patients must approve their release in writing, except as required by law.
  • Patients are provided to the degree known with complete information concerning their diagnosis, evaluation, treatment, and prognosis.
  • Patients are given the opportunity to participate in decisions involving their health care, except when such participation would cause life-threatening illness or injury to the patient or other individuals.

Patients have the right to information concerning:

  • Patient rights, responsibilities, and conduct.
  • Available services at University Health Services.
  • Instructions for after-hours and emergency care.
  • Fees for services at UHS.
  • Payment policy and options.
  • Patient's right to refuse participation in prescribed treatment plan.
  • UHS provider credentials.
