Our program appreciates your interest in becoming a mentor to a STRIVE U student. By signing below, you attest to the truthfulness of all information listed on this application. You agree to let our program confirm all information.

Research has shown that in mentoring relationships, it is better never to have started a relationship than to start one and stop before a mentor’s commitment is up. Do you understand the commitment it takes to be a mentor, and are you willing to accept it?

I promise that I shall hold in confidence all information regarding students involved in the USM STRIVE U Mentoring Program. I will not violate the confidential relationship between the program, student and volunteers. The exceptions to confidentiality are imminent danger to self or others: physical-sexual-emotional abuse. If any of these conditions occur, I will notify the USM STRIVE U Student Coordinator.
