Faculty Handbook Fundraising Prior to fundraising, CONHP faculty must fill out the Fundraising Request Form (included in the Faculty Handbook) with fundraising information for approval from the CONHP Dean. Please send this form to your CONHP Financial Administrator, Pam Hartford, for processing. Also, the Faculty in charge should set up a meeting to go over the fundraiser transactions with Pam Hartford before starting the fundraiser. A copy of the Fundraising Request Form will be returned to you after it is approved. Students who are raising money through a USM recognized student group should follow the Student Senate, Board of Student Organizations guidelines. All fundraising transactions for these groups are processed through the Student Senate Department instead of CONHP. Donations Prior to soliciting donations, CONHP faculty must fill out the Pre-Contact Approval Form (included in the Faculty Handbook) with donation information, to be approved by the CONHP Dean and the Advancement Department. Please attach a copy of the donation letter to be used and send this form to your CONHP Financial Administrator, Pam Hartford, for processing. A copy of the Pre-Contact Approval Form will be returned to you after it is approved. In-Kind donation forms for items should be filled out and submitted to Pam Hartford if the donor requests a donation letter from USM advancement Department for tax purposes. Receipts are available from your Financial Administrator. The donor should receive the original, and a copy should be returned to Pam Hartford with the cash or check made out to USM.
