Request Form to Add a Class Member in Blackboard

This form is to be used to add teacher's assistants, co-instructors, and course builders to a Blackboard course. 

Students who are not teacher's assistants may not be added using this form.  To add a student to a class that they have not registered for, please email the Registrar's Office for permission.
