NOTE: You may include a signed or anonymous Experiment Evaluation Form any time you want to provide feedback about an experiment. Download the form, print and complete it, and put it in the stack of lab reports handed in that day. If you wish it to be anonymous, do not put your name on the form, and do not attach it to your report.
